Pharmacodynamics and pharmacokinetics

The drug is a combination drug for the treatment of acute respiratory infections. Its properties are determined by its constituent components:

  1. Paracetamol is an analgesic-antipyretic, has an analgesic and antipyretic effect. The substance affects the thermoregulation center in the hypothalamus, relieves headaches, and lowers temperature.
  2. Vitamin C – ascorbic acid, takes part in regulating oxidation-reduction processes and carbohydrate metabolism. It affects blood clotting, tissue regeneration, participates in the synthesis of collagen, procollagen, glucocorticosteroids, and normalizes capillary permeability. Vitamin C stimulates the immune system and increases the body's resistance.
  3. Pheniramine - part of the group of histamine receptor blockers, eliminates lacrimation, itching, and redness of the eyes. The component reduces rhinorrhea, sneezing, and a feeling of nasal congestion.

Once inside, the components of the drug are quickly and completely absorbed, reaching maximum concentration levels in the blood plasma after 30-60 minutes. All active substances are well distributed in tissues and are found in physiological fluids and the hematoencephalic membrane of the brain. The drug slightly binds to plasma proteins, is metabolized in the liver to form inactive metabolites, and is excreted by the kidneys.

Indications for use

Fervex can be used in the treatment of acute respiratory and viral infectious diseases, which are accompanied by nasal congestion, elevated body temperature, sneezing and red eyes. Powder therapy is symptomatic. You can also use the drug to treat nasopharyngitis, to increase resistance to colds.

special instructions

When taking a powdered product, it is possible to distort laboratory test results when quantifying the level of glucose or uric acid in the blood plasma. Other circumstances that should be taken into account when treating with the drug:

  1. If the patient is being treated with Metoclopramide, Cholestyramine, Domperidone, a doctor's consultation and approval must be obtained before taking the powder.
  2. During therapy, alcohol consumption is prohibited; this will help avoid toxic liver damage. For persons prone to chronic use of ethanol, the drug is prohibited. If patients develop alcoholic hepatosis, the use of the medication may cause liver damage.
  3. Long-term use of Fervex in increased doses disrupts the peripheral blood picture.

pharmachologic effect

Combined drug for symptomatic treatment of acute respiratory infections. Paracetamol is an analgesic-antipyretic, has an analgesic and antipyretic effect, which is associated with its effect on the thermoregulation center in the hypothalamus; eliminates headaches and other types of pain, reduces fever. Ascorbic acid (vitamin C) is involved in the regulation of redox processes, carbohydrate metabolism, blood clotting, tissue regeneration, in the synthesis of corticosteroids, collagen and procollagen; normalizes capillary permeability. Increases the body's resistance, which is associated with stimulation of the immune system. Pheniramine is a histamine H1 receptor blocker that reduces rhinorrhea, nasal congestion, sneezing, lacrimation, itching and redness of the eyes.

Drug interactions

The combination of the powdered drug with certain medications is prohibited due to the manifestation of negative symptoms. Drug interactions:

  1. Antidepressants, antiparkinsonian drugs, antipsychotics, phenothiazine derivatives increase the risk of developing dry mouth, constipation, and urinary retention.
  2. Combining the drug with glucocorticosteroids increases the risk of glaucoma.
  3. Phenytoin, tricyclic antidepressants, ethanol, phenylbutazone, barbiturates, rifampicin can increase the production of hydroxylated metabolites of paracetamol, which can lead to severe intoxication.
  4. Cimetidine reduces the risk of hepatotoxicity from paracetamol.
  5. When combined with ethanol, the risk of developing acute pancreatitis and increased sedative effect increases.
  6. Paracetamol increases the effect of uricosuric drugs.

Interaction of Fervex with other drugs and special instructions

Pheniramine, which is in solution, interacts well with antihistamines, greatly enhancing all sedative functions. However, it is not recommended to use Fervex together with antidepressants, medications containing phenothiazines and those that help treat Parkinson's, since symptoms such as urinary retention, dry throat and constipation appear as side effects.

Taking the drug together with glucosteroids greatly increases the chances of developing glaucoma.

Using it with ethanol can lead to acute pancreatitis, and in combination with inducers of microsomal oxidation in the liver, intoxication occurs even when taking a very small amount of the drug. However, microsomal oxidation inhibitors together with Fervex may help reduce the risk of developing liver damage.

In addition, Fervex completely suppresses the effect of all drugs that have a uricosuric effect.

In no case should it be combined with alcohol, since the risk of developing liver damage in such people is simply enormous. Patients with diabetes should also use the drug with caution, since it contains a large amount of sugar. Although at the moment a powder is sold that is produced completely without it.

The use of Fervex is also undesirable in combination with other drugs containing paracetamol, since there is a possibility of overdose.

Overdose

When using the indicated doses, the drug is well tolerated. Overdose is possible if the dose of paracetamol is exceeded. It is manifested by pale skin, decreased appetite, and vomiting. When taking 10-15 g of the substance, hepatonecrosis, increased activity of liver transaminases, and an increase in prothrombin time are observed. Liver damage begins within 1-6 days.

Rarely, complications appear immediately, liver failure and tubular necrosis of the kidneys develop. In the first 6 hours after taking the increased vine, you need to perform a gastric lavage, introduce sh-group donors and precursors for glutathione synthesis (methionine after 8-9 hours and N-acetylcysteine after 12). Further administration of antidotes is determined by the concentration of paracetamol in the blood.

The best analogues

If it is necessary to replace Fervex, its analogues are used. The most effective and popular of them:

  1. Theraflu. The drug is produced by a pharmaceutical company from Switzerland. The product contains paracetamol, pheniramine and phenylephrine. Medicine for flu and colds in powder form for preparing a hot drink.

  2. Flucoldin. Domestic analogue of Fervex. Contains the same active ingredients and is available in powder form. Inexpensive and effective remedy.
  3. Antiflu. An imported product from the USA based on paracetamol, phenylephrine and chlorpheniramine. Has a lemon taste. It helps to cope well with colds and symptoms of acute respiratory infections.

There are also paracetamol-based products in tablet form (Antigrippin, Apap S Plus), which are also used for symptoms of colds and flu. The attending physician must prescribe this or that remedy.

Fervex contraindications

The medication is prescribed with caution for angle-closure glaucoma, liver failure, prostate hyperplasia, congenital hyperbilirubinemia syndromes (Gilbert, Rotor, Dubin-Johnson), alcoholic or viral hepatitis, and in old age. Contraindications for taking the medicine, according to the instructions, are:

  • exacerbation of stomach ulcers or erosions;

Analogs

Drugs with the same treatment effect or a similar composition can replace medications. Popular analogues of the drug are:

  • Bioparox is a dosed spray for inhalation based on fusafungine, has a local anti-inflammatory effect;

  • Coldrex - tablets, syrup and powder based on paracetamol, ascorbic acid, caffeine, phenylephrine and terpin hydrate.

Which is better – Fervex or Theraflu

Both drugs are similar in composition: Theraflu contains paracetamol (325 mg) and pheniramine (20 mg), but instead of vitamin C, phenylephrine (10 mg) is added. The dosage of active substances is also different; Theraflu contains less paracetamol, so it is safer. Otherwise, the medications are similar: they have antipyretic and analgesic effects and are used to treat colds.

"Fervex" or "Teraflu"

The drugs are almost similar, the difference is only in one component. If the product in question contains vitamin C, then Theraflu contains phenylephrine. The dosage of the components also varies. The analog contains slightly less paracetamol - 325 mg. Otherwise, it can be argued that the action is exactly the same.

Fervex is considered more dangerous for use by people at risk. Instructions for use (analogues contain less paracetamol) indicate a fairly extensive list of side effects and risks. The indications for the use of both drugs are the same. Some relieve fever, fight inflammation and eliminate pain. Both are intended for the treatment of colds.
