Official medical statistics indicate that peptic ulcer disease is becoming more common every year. A successful fight against such a serious disease can only be carried out with highly effective medications, which include Pilobact.

The product consists of several drugs that provide complex treatment and suppresses the activity of the specific microorganism that causes the ulcer.

Instructions for use

Pilobact is a drug with a pronounced antiulcer and antimicrobial effect. The combined effect of the following drugs included in its composition guarantees high eradication of Helicobacter pylori:

  • Omeprazole - reduces the production of gastric acid, regardless of the nature of the irritant. After taking it, the substance begins to act within an hour.
  • Clarithromycin is an antibiotic belonging to the macrolide group, active against Helicobacter pylori. Suppresses protein synthesis in microorganisms.
  • Tinidazole is capable of damaging the DNA of a pathogenic microorganism and inhibiting its synthesis. It is characterized by high activity against anaerobes, amoebas, Trichomonas, Giardia, and Helicobacter pylori.

Mode of application

Each Pilobact blister contains a set of drugs intended for one day of treatment. Tablets and capsules are swallowed whole without breaking or chewing. All three components are taken twice a day before meals, 1 pc.

After a week's course of treatment, doctors often prescribe taking Omeprazole (20 mg) for another 3 weeks.

Before starting a course of treatment, be sure to take into account the following features of the drug:

  • Before using the medicine, malignant processes are completely excluded;
  • The drug is limited in use for people with organic lesions of the nervous system;
  • While taking the drug, drinking alcohol is unacceptable to avoid the occurrence of a disulfiram-like reaction;
  • When used in parallel with substances that are utilized in the liver, the drug is used especially carefully;
  • When taken together with indirect anticoagulants, prothrombin time is monitored;
  • Thiadazole can turn urine dark.

While following the course of treatment, you need to be especially careful when driving a car or doing potentially dangerous work that requires quick reaction and concentration.

Release form, composition

Pilobact is available in the form of capsules and tablets, packaged in strips containing the daily dose for administration. The package includes 7 blisters. Each strip, calculated for the day of treatment, contains:

  • Omeprazole (20 mg) – 2 capsules;
  • Clarithromycin (250 mg) – 2 tablets;
  • Tinidazole (500 mg) – 2 tablets.

Reviews

There are a lot of reviews about Pilobact online, most of which are positive. Despite the rather high price of the drug, its high efficiency and successful release form with a dosage calculated for the course of treatment are noted.

Most people experienced improvements after the first days of taking it, and by the end of the week's course there was no doubt about the effectiveness of the product. In rare cases, side effects were observed that disappeared after discontinuation of use. Reviews from people who have experience in treatment with Pilobact can be read at the bottom of the article.

Pilobact is a highly effective remedy for combating stomach and duodenal ulcers. It consists of three active components, which are taken separately at the same time. One of them prevents the formation of hydrochloric acid in the stomach, and the other two have antimicrobial properties.

Pharmacological properties of the drug

The therapeutic effect of the drug Pilobact is based on its active ingredients.

Omeprazole suppresses the production of hydrochloric acid, reduces the increased acidity of gastric juice and maintains its therapeutic effect for 24 hours.

Clarithromycin is an antibiotic from the group of macrolides of semi-synthetic origin with a broad spectrum of action. This drug has a detrimental effect on the bacterium Helicobacter pylori, and also exhibits high therapeutic efficacy against most gram-positive and gram-negative bacteria.

Tinidazole inhibits bacterial synthesis and disrupts the DNA structure of the infectious agent.

Types of medicine Pilobact: NEO and AM, application features

Medicines Pilobact AM and NEO are combination drugs for the treatment of ulcers of the digestive tract. In most cases, the disease is caused by the proliferation of the Helicobacter bacteria. You can get rid of it only through complex therapy, which involves the use of several medications.

The instructions for use position both Pilobact as highly effective antiulcer agents that have antimicrobial, antisecretory and antibacterial effects.

NEO

This drug is presented in a pack of 6 tablets. Each combination consists of a daily dose. In the package the consumer will find:

  • omeprazole (40 mg) – 2 capsules, which effectively reduces the acidity of stomach juice;
  • clarithromycin (1000 mg) – 2 capsules of an antibacterial agent belonging to the macrolide group;
  • tinidazole (1000 mg) – 2 capsules of an antimicrobial drug from the antiprotozoal group.

AM

The medicine Pilobact AM is an antiulcer drug, presented in capsules in the amount of 8 pieces. It differs somewhat from its predecessor in active ingredients. It includes:

  • omeprazole (40 mg) – 2 tablets of an antisecretory agent;
  • clarithromycin (1000 mg) – 2 broad-spectrum antibiotic tablets;
  • amoxicillin (2000 mg) – 4 tablets of a penicillin antibacterial agent.

Operating principle

The triple therapeutic effect of Pilobact drugs allows you to completely destroy the proliferation of Helicobacter bacteria, thereby eliminating the negative effects of microorganisms and manifestations of peptic ulcer disease.

Omeprazole, which is part of both medications, quickly reduces the acidity of the stomach contents and eliminates the negative effects of pepsin. The active substance normalizes digestive function. The effect is observed within an hour after use and lasts throughout the day.

Clarithromycin is the main medicinal substance of the drug Pilobact. This antibiotic is effective against many microorganisms; it has a wide spectrum of action and a delayed effect. After using clarithromycin, its effectiveness remains for several days.

Interesting! Features of taking the drug Noflux - cheap analogues

Amoxicillin, part of Pilobact AM, is a semi-synthetic antibacterial agent. This substance shows high effectiveness against the Helicobacter bacteria.

Tinidazole, replacing amoxicillin in a similar drug, has antiprotozoal activity.

It is effective against amoebas, Giardia, Helicobacter bacteria, and in combination with the described substances it complements the therapeutic effect.

Contraindications and adverse reactions

Pilobact AM instructions prohibit use in case of hypersensitivity to any of the components. It is unacceptable to carry out therapy during pregnancy, breastfeeding, and in childhood. Treatment of ulcers with triple medication is contraindicated in cases of damage to the central nervous system, porphyria, depression of cerebral circulation, liver or kidney failure.

The following side effects may occur due to the use of Pilobact:

  • allergies in the form of skin rash or swelling;
  • digestive dysfunction (nausea, vomiting, abdominal pain);
  • neurological manifestations (headaches, excitability, sleep disturbance);
  • changes in blood and liver parameters.

Important! If your health worsens due to the use of the medication, you should immediately seek medical help.

Instructions

Pilobact AM tablets are designed for a weekly course of treatment. The package contains 7 strips, each of which contains a daily dose of the drug, divided in two.

Take the drug from the NEO line, 3 capsules in the morning and evening. Another type of drug is used in 4 tablets. The pills should be consumed without preliminary crushing, in one go. You can only drink clean water. To enhance the effect, it is recommended to take the medicine 1 hour before a meal or only 2 hours after a meal.

Additionally

Many patients wonder: are there any analogues of Pilobact? Potential consumers want to replace the drug in order to save money. To date, there are no absolute substitutes for this drug. For the drug Pilobact, an analogue can only be selected by independently combining antibacterial, antiprotozoal and antisecretory active ingredients.

It is unacceptable to drink alcohol during treatment with an antiulcer drug. Ethanol not only has a detrimental effect on the condition of the gastric mucosa, but can also negatively affect the work of the antibiotic. People who combine antiulcer drugs and alcohol develop disulfiram-like reactions.

Interesting! What is “Epigastralgia” - features of diagnosis and treatment

When using the medication, it is necessary to take into account that tablets may affect the effectiveness of other drugs:

  • reduce the protective function of oral contraceptives (you need to use additional barrier agents);
  • reduce the absorption of ketoconazole and ampicillin;
  • reduce the metabolic rate when treated with diazepam and phenytoin;
  • increase the concentration in the body of digoxin, lovastatin, cyclosporine.

Important! If the patient takes any pharmaceutical products, then the doctor must be informed about this when prescribing anti-ulcer tablets.

Finally

Pilobact preparations have shown themselves to be good. They are highly popular in gastroenterology and receive positive reviews from patients.

The only drawback of the medication is its high cost. Before using the medicine, you should definitely consult a specialist.

Contraindications

The drug must be prescribed by a doctor after a preliminary examination and tests. Before starting to use the drug, you should read the accompanying instructions in detail, since tablets and capsules have a number of limitations and contraindications:

  • Individual intolerance to the components of the drug;
  • Severe allergic reactions to antibiotics in the past, especially to drugs from the group of macrolides and penicillins;
  • Pregnancy and breastfeeding period;
  • Severe diseases of the central nervous system;
  • Dysfunction of bone marrow hematopoiesis;
  • Age under 18 due to lack of experience and unproven safety of the drug;
  • Severe liver and kidney damage, acute renal failure.

Directions for use and dosage regimen

The patient uses 1 blister from a cardboard box per day, which contains the required daily dose of medication. At one time you should take 1 tablet and 1 capsule from the pack - in the morning and in the evening. It is not recommended to crush or open tablets and capsules; they must be swallowed whole, since the drug must dissolve directly inside the digestive tract. Take medications with plenty of water.

The duration of treatment with the drug is 1 week.

Use of the drug during pregnancy and breastfeeding

The drug Pilobact is not prescribed for the treatment of pregnant women, since the safety of drugs regarding intrauterine development of the fetus has not been established and there is no experience of use.

This drug is not prescribed for the treatment of peptic ulcers in nursing mothers, since the active substances included in the tablets and capsules are excreted in breast milk and can negatively affect the baby. If therapy is necessary, the woman is advised to stop lactation.

Pharmacokinetics

All three drugs included in Pilobact® have good absorption when taken orally.

Omeprazole is rapidly absorbed after oral administration, its bioavailability is 30–40%. Food intake does not affect the bioavailability of omeprazole. Cmax is reached within 0.5–1 hour. The binding to plasma proteins is 90%. Almost completely metabolized in the liver. The main route of excretion is in the urine (80%).

Clarithromycin is rapidly absorbed from the gastrointestinal tract. The absolute bioavailability of 250 mg clarithromycin is approximately 50%. Eating slightly slows down the onset of absorption of clarithromycin and the formation of 14-hydroxyclarithromycin, but does not affect bioavailability. When taken on an empty stomach, peak serum concentrations are achieved within 2 hours after oral administration and are 0.6 and 0.7 mcg/ml for clarithromycin and its main metabolite. T1/2 of clarithromycin is 3–4 hours. Clarithromycin is widely distributed in the body. The concentration of clarithromycin in tissues exceeds that in serum. Protein binding ranges from 42 to 70%. It is excreted by the kidneys and in feces (20–30% in unchanged form, the rest in the form of metabolites). The simultaneous administration of clarithromycin and omeprazole improves the pharmacokinetic properties of clarithromycin: the average Cmax value increases by 10%, the minimum concentration by 15% compared with the same indicators with clarithromycin monotherapy. The concentration of clarithromycin in the gastric mucosa is also increased when administered simultaneously with omeprazole.

The bioavailability of tinidazole is about 100%. Communication with plasma proteins 12%. Time to reach Cmax is 2 hours. Penetrates through the blood-brain barrier and placenta. Excreted in breast milk. Metabolized in the liver to form pharmacologically active hydroxylated derivatives, which can enhance the effect of tinidazole. T1/2 is 12-14 hours. 50% is excreted in the bile, 25% unchanged in the kidneys and 12% in the form of metabolites. It is reabsorbed in the renal tubules.

Side effects

During treatment with Pilobact, patients often develop adverse reactions:

  • From the digestive system - nausea, abdominal pain, vomiting, lack of appetite, bloating, dry mouth, unpleasant taste in the mouth, impaired liver function, increased activity of liver transminases, development of enterocolitis;
  • From the nervous system - lethargy, confusion, dizziness or headaches, emotional lability, increased fatigue, psychosis, disorientation in space, tinnitus;
  • Muscle weakness, pain in muscles and joints;
  • From the blood system - decreased levels of platelets and leukocytes in the blood, thrombocytopenic purpura, anemia;
  • Allergic reactions - rashes, skin itching, redness, urticaria, development of angioedema or anaphylaxis;
  • In rare cases, fever, gynecomastia, bronchospasm, swelling of the extremities, and interstitial nephritis occur.

Release form and composition

Pilobact has a rather unusual release form.

The cardboard packaging contains instructions with a detailed description of all additional information, as well as foil blisters containing medications. The quantity of these blisters is designed for a 7-day course of treatment.

In each of them you can find the presence of 2 capsules and 4 tablets. This amount of medicines should be used by a person during the day.

Capsules are presented:

  • Omeprazole with a dosage of the main active ingredient of 20 mg.
  • And also tablets, 2 of which are 250 mg of Clarithromycin.
  • 2 tablets of Tinidazole, 500 mg of the main active ingredient in each tablet.

It can also be noted that there are additional components that do not have an effect in the treatment of pathology.

pharmachologic effect

Pilobact promotes the development of antiulcer action.

Pharmacodynamics

Pilobact, due to its composition, contributes to the creation of not only antiulcer manifestations, but also the development of antimicrobial action.

  • Omeprazole helps reduce secretion and acid-forming function in the stomach. This occurs due to the inhibition of special enzymes that are contained in the cells that make up the membrane of parietal cells on the mucous membrane of the stomach wall. This development contributes to the fact that stimulated and basal secretion decreases. As a result of oral administration, Omeprazole causes the secretion of gastric juice to decrease after one hour from the moment of use. The effect may take up to 24 hours. If you repeat the drug within 24 hours, the decrease in basal secretion will reach almost 100% of the result.
  • Due to Clarithromycin, which is a semi-synthetic derivative of the erythromycin group , also included in the class of macrolides, it influences the effect in relation to Helicobacter pylori infection, anaerobic microorganisms, gram-negative and gram-positive representatives of microflora.
  • Due to the third component - Tinidazole , which has an effect on lamblia, amoebas, trichomonas and Helicobacter, there is an antiprotozoal effect.

Pharmacokinetics

All components that make up Pilobact have a high degree of absorption capacity. All of them bind to plasma proteins and are metabolized by liver cells.

They are excreted mainly through urine, and to a small extent through feces. Their simultaneous use with food can lead to a decrease in the severity of the effect and delayed absorption.

Indications for use

  • Pilobact is used in the treatment of gastric or duodenal ulcers, which are associated with Helicobacter pylori infection.

In this case, Pilobact will represent eradication therapy.

"Honey Spas" from parasites and worms

I used to be like a sleepy fly all the time: my head hurt, I felt weak, I couldn’t sleep in the evening. and wake up in the morning. It’s strange, but my gynecologist advised me to try Honey Spas, suspecting infection with parasites due to constant inflammatory “female-like” ones. I took 2 courses, and now the energy is pouring out of me! And the gynecological problems also went away! Anna,

Contraindications

Before you start using Pilobact, you must undergo a mandatory consultation with a specialist to rule out possible consequences for the body.

In addition, additional laboratory diagnostics may be required, which will include blood tests or biochemical parameters.

Eliminating contraindications reduces the risk of complications or possible adverse symptoms.

Among the most common contraindications are:

  • The presence of individual intolerance to any component included in the composition.
  • A history of cases with possible allergy to antibacterial drugs, in particular in relation to macrolides and penicillins.
  • State of pregnancy or possible period of breastfeeding.
  • Serious pathologies of the central parts of the nervous system.
  • Disturbances in the functional state associated with inhibition of bone marrow hematopoiesis.
  • Severe pathologies of the urinary and hepatobiliary systems, which are accompanied by the development of functional impairment.

Due to the absence of possible toxic reactions to the developing organism, Pilobact is prohibited for use by patients under the age of 18 years.

Directions for use and dosage

  • Pilobact is taken internally.
  • It is recommended to take it simultaneously with food.
  • A single dosage should include 1 tablet of Omeprazole, 1 tablet of Clarithromycin and 1 tablet of Tinidazole.
  • This combination is used twice a day at the same hours.
  • The duration of therapy is a week.

Pregnancy and breastfeeding

Due to the lack of information about the effect of Pilobact on the body, its use during pregnancy is prohibited.

Absorption into the internal environment of the body allows the drug to pass through breast milk to the baby, resulting in subsequent negative symptoms.

That is why, if it is necessary to treat a peptic ulcer, it is necessary to temporarily stop breastfeeding until treatment stops and Pilobact is completely removed from the body.

Application for children

Pilobact is prohibited for use by children.

Use for liver dysfunction

The use of Pilobact with substances metabolized in liver cells should be limited. This is due to the fact that there is an increase in the negative impact on the organ with the possible onset of acute liver failure.

Side effects

These reactions develop mainly in patients who violate the dosage regimen or rules for using Pilobact.

A similar condition appears:

  • Damage to the organs of the digestive system with nausea, vomiting, abdominal pain, lack of appetite, bloating, constant unpleasant taste in the mouth, damage to liver cells with disruption of their functional activity.
  • Disorders affecting the nervous system are manifested by headaches, lethargy and confusion, dizziness, signs of emotional lability, increased fatigue, spatial disorientation and tinnitus. The development of one type of disorder indicates the patient’s tendency to develop acute psychosis.
  • , pain in muscle tissue and joint apparatus gradually increases
  • There is a decrease in the functional activity of the platelet and leukocyte units , which can manifest as thrombocytopenic purpura and anemia.
  • Allergic reactions are characterized by the appearance of polymorphic rashes, redness and itching of the skin, and an anaphylactic state and angioedema may also develop.
  • Less commonly, one can note the appearance of gynecomastia , mild bronchospasm, as well as swelling of the extremities and interstitial nephritis.

special instructions

  • Before prescribing Pilobact, the patient must undergo a full clinical examination for the presence of oncological pathologies in the gastrointestinal tract. This is due to the fact that under the influence of Pilobact there will be a decrease in the severity of the clinical picture, with the continued growth of the tumor.
  • Tinidazole leads to a change in urine color , its shade becomes darker, which in some cases alarms the patient.

Overdose

If a patient takes Pilobact for more than seven days and takes Pilobact several times more than the permissible dosage, the risk of developing an overdose increases, which is expressed by:

  • Development of nausea, vomiting and headaches.
  • The appearance of disturbances in the water-salt balance, developing as a result of diarrhea and incessant vomiting.
  • Characteristic disturbance of consciousness and weakness.
  • Changes in the functioning of the liver with insufficient performance of its function and possible disruption of the enzyme composition.

If such symptoms occur during the use of Pilobact, you must immediately stop taking the drug.

If possible, use products from the group of enterosorbents, in cases where no more than 2 hours have passed from the moment of administration, it is advisable to perform gastric lavage.

To reduce clinical manifestations, symptomatic therapy is required; in some cases, hemodialysis may be required. The patient needs to contact a specialist to decide on further treatment.

Drug interactions

In order to exclude the development of side effects or negative consequences in the form of complications, it is necessary to take into account the possible combination with other drugs used by the patient.

Among them are to consider:

  • Theophylline used by the patient in combination with Clarithromycin contained in Pilobact leads to an increased effect and accumulation of the first substance in the body.
  • Combination with enveloping agents, antacids and enterosorbents leads to the fact that Pilobact is absorbed in smaller quantities and, therefore, does not contribute to the manifestation of the desired effect.
  • Ampicillin, Warfarin and Phenytoin have less effect.
  • Tinidazole helps to increase the severity of the effects of indirect anticoagulants. This increases the risk of bleeding.

Pilobact and alcohol

The combination of Pilobact and alcohol is prohibited. Simultaneous use of alcohol leads to increased toxic effects on the liver.

Effect on driving

Due to a possible negative reaction from the nervous system, in the form of dizziness, disturbances of consciousness, etc., while taking and removing the substance from the body, it is recommended to stop using Pilobact.

Price

The average cost of Pilobact ranges from 1000 rubles to 1200 rubles .

Storage conditions and expiration dates

Pilobact is good for two years from the date of manufacture. It should be stored in places inaccessible to children, at a temperature not exceeding 25 degrees.

Analogs

There are no absolute analogues of Pilobact, but the effect can be achieved by combining several medications similar to those that are included in its composition.

Among them:

  • Gastracid . A drug based on agents that help reduce acidity. It contains hydroxide components. The average cost is from 100 to 150 rubles .

Overdose

If you independently exceed the recommended dose or use the drug for more than 7 days, the patient develops signs of overdose:

  • Nausea, vomiting, headaches;
  • Violation of water-salt balance due to diarrhea and vomiting;
  • Confusion and weakness;
  • Severe damage to liver function.

If such symptoms appear, the patient must immediately stop treatment with the drug, rinse the stomach, and administer enterosorbents orally. If necessary, hemodialysis and symptomatic treatment are performed.

Interaction with other drugs

The drug is not recommended to be prescribed to patients simultaneously with Theophylline, since under the influence of Clarithromycin the concentration of Theophylline in the blood plasma increases.

It is not recommended to take enveloping agents, enterosorbents and antacids simultaneously with the drug Pilobact, as this leads to a decrease in the therapeutic effect of the drug.

Under the influence of Omeprazole, the absorption of Ampicillin, Warfarin and Phenytoin may slow down.

Under the influence of Tinidazole, the therapeutic effect of indirect anticoagulants is enhanced, which can lead to the risk of bleeding.

special instructions

Before prescribing the drug Pilobact, patients should undergo a detailed examination and exclude oncological diseases of the digestive tract, since the clinical picture of cancer can be masked under the influence of drugs.

The drug is prescribed with special caution to patients simultaneously with drugs that are metabolized in the liver. Massive effects of drugs on liver tissue can lead to suppression of organ function and the development of acute liver failure.

During treatment with the drug, the patient's urine may turn dark, which is due to the influence of Tinidazole.

Since patients may experience dizziness and disturbances of consciousness while taking medications from a pack, they should refrain from driving vehicles and operating complex equipment that requires increased concentration during the period of therapy.

Pilobact

Pilobact in various forms is a drug that is widely used in gastroenterology in the treatment of stomach and intestinal ulcers caused by Helicobacter infection.

To ensure the rationality of therapy, it is important to take the medicine in accordance with the requirements contained in the instructions for use.

Patient reviews

Ilya V.: the convenient form of release (combi-packaging) allows you to correctly take the drug, which is excellent for helping with ulcers in the stomach and intestines. Prescribing Pilobact AM simply saved me from the disease. In one week it was possible to achieve a significant improvement in the condition of the stomach. To prevent relapses, I continue treatment with Omez.

Victoria A.: Pilobact is much more effective than its analogues, which may cost less. Due to the fact that this Pilobact has a complex composition, it is possible to quickly remove Helicobacter with a minimum of side effects that are often encountered when taking other groups of drugs.

Reviews from medical specialists

Oleg Vantsenko, gastroenterologist: I often prescribe Pilobact or Pilobact AM to patients with ulcers who have severe pain in the stomach area. I would like to note that I am always very pleased with the effectiveness of the drug. It is extremely rare that I choose other products that can replace Pilobact (the choice is very limited).

Irina Petrovskaya, gastroenterologist: the convenient dosage regimen and rational dosage regimen of Pilobact allow it to be widely used in the treatment of ulcers for a short time. The high efficiency and safety of the drug makes it indispensable in the treatment of this disease. Pilobact always allows you to quickly eliminate the disease.
