Pharmacology
The maximum presence of the active substance in the blood serum after taking a dose of the drug is observed after 5 hours. Bioavailability of the drug is approximately 20%. Course use does not have any serious effect on the process of drug absorption. The binding of statin to blood proteins reaches 90%. The absorption of Rosucard occurs in the liver tissues.
The drug is excreted naturally: 10% with urine, the remaining 90% with feces. The primary therapeutic effect is observed on the seventh day from the start of treatment. The maximum is achieved by the end of the second week of regular use.
Pharmacodynamics and pharmacokinetics
Rosucard belongs to the group of statins. It inhibits HMG-CoA reductase, the enzyme that converts HMG-CoA to mevalonate.
In addition, this drug increases the number of LDL receptors on hepatocytes, which increases the intensity of LDL catabolism and uptake and causes inhibition of VLDL synthesis, reducing the total content of VLDL and LDL. The drug reduces the concentration of LDL-C, high-density non-lipoprotein cholesterol, VLDL-C, TG, apolipoprotein B, VLDL-TG, total cholesterol, and also increases the content of ApoA-1 and HDL-C. In addition, it reduces the ratio of ApoB and ApoA-1, non-HDL-C and HDL-C, LDL-C and HDL-C, total cholesterol and HDL-C.
The main effect of Rosucard is directly proportional to the prescribed dosage. The therapeutic effect after the start of treatment is noticeable within a week, after about a month it becomes maximum, and then strengthens and becomes permanent.
The maximum concentration of the main active substance in plasma is established after 5 hours. Absolute bioavailability is 20%. The degree of binding to blood plasma proteins is about 90%.
With regular use, the pharmacokinetics do not change.
Rosucard is metabolized through the liver. Penetrates well through the placental barrier. The main metabolites are N-dismethyl and lactone metabolites.
The half-life is approximately 19 hours, and it does not change if the dosage of the drug is increased. Plasma clearance on average is 50 l/h. Approximately 90% of the active substance is excreted unchanged through the intestines, the rest through the kidneys.
Gender and age do not affect the pharmacokinetics of Rosucard. However, it depends on race. In Indians, the maximum concentration and mean AUC value are approximately 1.3 times higher than in Caucasians. AUC in people of the Mongoloid race is 2 times higher.
Indications for use
Indications for use of Rosucard:
- hypercholesterolemia (primary) and dyslepidemia (mixed) - the drug is prescribed as a supplement to the prescribed therapeutic diet;
- atherosclerosis - allows you to slow down the progression of pathology, complements the therapeutic diet, allows you to reduce LDL (low-density lipoproteins) to a physiological level;
- familial homozygous hypercholesterolemia – Rosacard is part of the treatment.
Statin can be used as a prophylactic agent to prevent the development of complications of cardiovascular pathologies, as well as the high likelihood of atherosclerosis.
Indications for use of Rosucard
The use of Rosucard is indicated for the following diseases:
- Primary heterozygous (hereditary) hypercholesterolemia according to Fredrickson. In addition to drug therapy, the patient is recommended a low-carbohydrate diet and special exercise to reduce body weight.
- Hereditary homozygous hypercholesterolemia.
- Insufficiency of lipoprotein lipase, which was formed against the background of a defective enzyme and activator of apoprotein CII. This condition often occurs with diabetes.
- As an inhibitor of lipid metabolism disorders and accompanied by the deposition of cholesterol on the inner walls of blood vessels. Therapy with Rosucard helps reduce cholesterol and LDL cholesterol.
The drug is widely recommended for the prevention of cardiovascular diseases (myocardial infarction, stroke, angina, persistent arterial hypertension).
Rosacard also helps patients who have undergone arterial revascularization. Tablets are not prescribed if the patient has symptoms of acute ischemia.
Contraindications
The use of a statin should be discontinued in the following cases:
- individual intolerance to the component composition;
- severe kidney damage;
- acute form of liver pathologies;
- myopathy;
- gestation;
- lactation.
Important! The medication should not be prescribed to women of fertile age who do not take or use contraceptives.

Directions for use and dosage
According to the instructions for use, the tablets are taken whole with the required volume of clean water. You can drink them at any time, without focusing on food intake. How long to take Rosucard is decided by the attending physician. Before starting a course of treatment, the patient must follow a special lipid-lowering diet that helps reduce bad cholesterol levels, and continue to adhere to the principles of dietary nutrition throughout the course.
The dosage of the product is calculated individually. The basis is taken from test indicators that characterize the patient’s current condition. If a person has been prescribed a dosage of 5 mg, then the tablet at a dosage of 10 mg must be divided into two parts. The initial dosage (recommended) is 5/10 mg per day.
Used at the beginning of treatment or in patients who started taking Rosucard as an alternative drug after stopping another inhibitor. The choice of initial dosage is influenced by the current level of blood cholesterol, the likelihood of developing cardiovascular complications, and the risk of side effects.
The selected dosage may be revised upward after 4 weeks from the start of taking the statin. A daily dosage of 40 mg is prescribed in exceptional cases, as it often causes side effects. Indications include severe hypercholesterolemia and a high risk of developing cardiovascular complications. Patients in this case need constant medical supervision.
For liver failure
In case of liver dysfunction, which on the Child-Pugh scale has a score of less than seven points, no dose adjustment is needed.
For renal failure
In mild cases of pathology, no dose adjustments are made. The initial dose is 5 mg per day (1/2 tablet of 10 mg). With moderate severity, characterized by a creatinine clearance level of 30–60 ml/min, high doses of Rosucard (40 mg) are strictly contraindicated. Predisposition to the development of myopathy

Drug interactions
When starting a course of treatment, it is necessary to take into account the drug interaction of Rosucard with other medications.
- Cyclosporine. Helps increase AUC levels.
- Lipid-lowering drugs, in particular Gemfibrozil. The combined use increases the concentration of Rosucard, which leads to an increased risk of developing myopathy. The maximum permissible dose of Rosucard is 20 mg.
- Fibrates. The dose of Rosucard is 5...20 mg. Large dosages are prohibited.
- Protease inhibitors. They cause increased exposure of the active substance. This combination is not used in the treatment of patients with HIV infection.
- Erythromycin. When used together, there is a decrease in the maximum concentration of the active substance of Rosucard by approximately 30%.
- Lopinavir. Increases the AUC and maximum concentration of the active substance of the drug in question.
- Ezetimibe. It is possible that there may be mutual enhancement of side effects.
- Antacids containing aluminum/magnesium hydroxyl. Reduces Rosecard concentration by 50%. Therefore, at least two hours should pass between doses of the drug.
- Oral contraceptives. When using OCs in combination, it is necessary to monitor the woman’s well-being for the development of side symptoms.
Features of application
You can take this medicine only after consulting your doctor! Self-medication leads to undesirable consequences.
Pregnancy and lactation are a direct contraindication to taking statins. For proper development and growth of the fetus, an increased concentration of serum cholesterol is needed. When taking rosuvastatin, the risk to the fetus will be higher than the benefit to the mother.
To avoid the development of side effects of the drug in infants, it should not be taken during lactation. If it is not possible to refuse treatment, it is necessary to interrupt breastfeeding the baby.
