Effective nasal drops and sprays for allergies and allergic rhinitis in children and adults

As you know, allergic rhinitis (rhinitis) is represented by a complex of symptoms caused by allergic inflammation of the nasal mucosa, and sometimes the sinuses. These include nasal congestion, nasal discharge, itching, and sneezing. Previously, nose drops were used to combat these symptoms, but now nasal sprays with an antiallergic effect are becoming increasingly popular.

Types of nasal sprays

Nasal sprays against allergies (nasal comes from the Latin word nasis - nose) can be divided into several groups:

  • Hormonal sprays for allergies.
  • Antihistamines (non-hormonal allergy sprays).
  • Cromoglycates (cromoglycic acid preparations, non-hormonal).
  • Barrier (non-hormonal).
  • Combined.

Many websites provide lists of allergy sprays, where they suggest using vasoconstrictor allergy sprays. Vasoconstrictor sprays containing xylometazoline and similar substances actually alleviate the symptoms of allergic rhinitis, but they cannot be used for a long time; they are usually recommended to be taken for no more than a week, so they are not suitable as anti-allergic sprays.

Classification of sprays

In order to get rid of this disease or reduce the symptoms of its manifestation, it is necessary to choose the right remedy. The best way to help with this is a specialized doctor - an allergist. To combat allergic rhinitis, there are the following types of sprays:

  1. Combined. They contain several components that have vasoconstrictor and antihistamine effects.

  2. Steroids. They have antiallergic, anti-inflammatory and immunosuppressive properties. Facilitate the course of the disease.
  3. Hormonal. Used for severe forms of the disease, as well as for allergic conjunctivitis.
  4. Vasoconstrictors. Indispensable for swelling of the mucous membranes, when the nasal passage is completely blocked. These sprays can be short-term or long-acting.
  5. Antihistamines. The name itself suggests that these are anti-allergy drugs. They have a therapeutic effect at the site of their use.

Hormone-containing allergy sprays

There is a common prejudice against hormone-containing drugs, and sprays in particular. However, this is one of the most effective drugs against allergic rhinitis to date. Their effect is manifested mainly on the mucous membrane, and in normal doses they have virtually no effect on the body as a whole. Nasal sprays such as:

  • Flixonase;
  • Avamis;
  • Nasonex;
  • Desrinitis.

They help control both persistent and seasonal rhinitis of allergic origin. In addition, sprays with hormones are effective in the presence of allergic conjunctivitis or vasomotor rhinitis. They are used in severe or moderate cases, for a long time or only during the flowering of plants that are sources of allergenic pollen (then treatment begins 2 weeks before the expected unfavorable season).

The most common complication of hormonal sprays is nosebleeds, very rarely - perforation (formation of a hole) in the nasal septum. To avoid this, it is recommended to direct the spray stream towards the wing of the nose, that is, outwards, and not inwards (towards the nasal septum). Candidiasis of the mucous membranes of the nose and throat also sometimes develops.

Hormonal sprays for allergies

  • Flycosonase (contains fluticasone). A drug that has been used for many years and has proven itself. The main disadvantage is the price.

For adults and children over 12 years of age, for the prevention and treatment of allergic rhinitis, the recommended dose is 2 injections into each nasal passage once a day, preferably in the morning (total dose 200 mcg/day). Once symptom control is achieved, the dose can be reduced to 1 spray in each nostril once a day.

There is a cheaper analogue - nazarel.

  • Avamys (contains fluticasone furoate). The manufacturer considers it a new generation allergy spray; due to the unusual injection device and the modified formula of the active substance, the effect should occur faster and stronger.

To achieve maximum therapeutic effect, it is necessary to adhere to a regular regimen of use. The onset of action can be observed within 8 hours after the first administration. It may take several days to achieve maximum effect.

Adults and adolescents (12 years and older): The recommended initial dose is 2 sprays (27.5 mcg of fluticasone furoate per spray) into each nostril once a day (110 mcg/day). Once adequate symptom control is achieved, reducing the dose to 1 spray in each nostril once daily (55 mcg/day) may be effective for maintenance treatment.

  • Nasonex (contains mometasone). High quality spray. Considered the safest for children. There are also analogues: momat rhino, desrinit, nosephrine (Russian analogue). You must understand that all safety studies were conducted on the original drug.

Before using Nasonex® nasal spray for the first time, it is necessary to “calibrate” it by pressing the dispensing device 6-7 times. After “calibration”, a stereotypical delivery of the drug is established, in which with each press of the button approximately 100 mg of mometasone furoate suspension is released, containing mometasone furoate monohydrate in an amount equivalent to 50 μg of chemically pure mometasone furoate. If the allergy nasal spray has not been used for 14 days or longer, recalibration is necessary before using it again. Before each use, shake the spray bottle vigorously.

For adults (including the elderly) and adolescents over 12 years of age, the usually recommended preventive and therapeutic dose of the drug is 2 inhalations (50 mcg each) into each nostril, once (total daily dose 200 mcg). After achieving the desired therapeutic effect for maintenance therapy, it is advisable to reduce the dose to 1 inhalation in each nostril 1 time per day (total daily dose 100 mcg).

  • Tafen nasal (contains budesonide). Budesonide is a fairly weak hormone, but one of the safest and most studied. Therefore, the spray is more suitable for long-term use. Adults and children over 6 years of age: Initially, 2 doses of 50 mcg of budesonide in each nasal passage 2 times a day. The usual maintenance dose is 1 dose in each nasal passage 2 times a day or 2 doses in each nasal passage 1 time per day, in the morning. The maintenance dose should be the lowest effective dose that eliminates symptoms of rhinitis.
  • Nasobek (contains beclomethasone). Inexpensive anti-allergy spray. Before using Nasobek, the nasal passages must be free. Adults and children over 6 years of age: 1 dose (50 mcg) in each nasal passage 2–4 times a day (200–400 mcg). The dose is then reduced depending on the patient's response. The maximum daily dose is 400 mcg.

Spray for an allergic child

Parents should ask their pediatrician how to prevent and treat allergic rhinitis in children.

In children, the manifestation of allergic rhinitis occurs no less often than in adult patients. Therefore, parents should ask their pediatrician how to prevent and treat allergic rhinitis in children. You cannot choose a drug on your own, since only a doctor can evaluate the benefits and harms of a drug and choose the most gentle remedy in a particular case. To select a nasal spray, the doctor will take into account the child’s age and body weight, developmental characteristics, and the presence of negative reactions to any components of the medications. Sprays are prescribed when the baby is one year old. At a younger age, you can only use nasal drops.

Antihistamine and hormonal sprays are not prescribed for children under 2 years of age. To restore breathing through the nose, remove congestion and swelling, you need to use vasoconstrictors. The dosage should be minimal. These medications are: Naphazolin, Rinazolin, Otrivin baby, Nazol kids. At the same time, the nasal mucosa can be moisturized with Salin, Aquamaris spray. You can add traditional medicine to your therapy, but this is only after a visit to an allergist.

Children over 6 years old can use sprays: Alcedin, Cromohexal, Rinoklenil, Flucoticasone, Nasobek, Vibrocil, Nazaval, Allergodil. The regimen of use is discussed with the doctor, but usually the spray is used 3 times a day at regular intervals. Antiallergic drugs remove inflammation from the mucous membrane, relieve swelling, and reduce the amount of nasal discharge. In addition to therapy, sprays have a preventive value - they prevent potential allergens from getting into the nasal mucosa and into the bloodstream.

To summarize, it can be noted that the main task of an allergy spray is to eliminate unpleasant symptoms and improve the patient’s quality of life. There is only one condition - you should not prescribe the drug yourself - this is fraught with complications. You need to contact a specialist and follow his recommendations.

Antihistamine allergy sprays (H1 blockers)

Weaker than hormonal, especially with concomitant eye symptoms. And in general, weaker than hormonal ones. They have a significant advantage over antihistamine tablets - they act on the mucous membrane, and not on the entire body (although modern antihistamine tablets are well tolerated). The advantage over hormonal sprays is that they act relatively quickly, the effect begins within a few minutes. Also often used in combination therapy, together with hormonal allergy sprays.

The main side effects are local irritation of the mucous membrane, burning of the mucous membrane.

  • Tizin Alergy (contains levocabastine). A fast-acting allergy spray, the effect occurs within a few minutes after use. The manufacturer claims that it also affects eye symptoms - itchy eyes, red eyes, watery eyes. Relatively inexpensive. Not to be confused with just Tizin.

Administer intranasally to adults and children over 6 years of age, 2 doses (100 mcg) in each nasal passage 2 times a day. Use of the drug should be continued until symptoms resolve. For more severe symptoms, the drug can be used 3-4 times a day. Before first use, remove the protective cap and press the spray nozzle several times until a cloud of “fog” appears. The bottle is ready for further use. Inhale the drug through the nose.

  • Allergodil (contains azelastine). It appeared quite a long time ago, but did not gain popularity either among patients or specialists. Quite expensive. Many patients complain of an unpleasant bitter taste in the mouth after use.

For allergic rhinitis and rhinoconjunctivitis, adults and children 6 years of age and older are administered 1 dose (140 mcg/0.14 ml) into each nasal passage 2 times a day in the morning and evening. If necessary, adults and children over 12 years of age - 2 doses (280 mcg/0.28 ml) in each nasal passage 2 times a day in the morning and evening. Allergodil® is used until symptoms stop and is suitable for long-term use, but not more than 6 months of continuous treatment.

Antihistamine non-hormonal sprays

Drugs of this type act directly on histamine. It is produced during the fight between antigens and allergens, and causes all the unpleasant symptoms - itching, sneezing, swelling, pinching, redness, mucus in the nose.

An allergy spray with an antihistamine effect suppresses the release of the substance, and as a result, symptoms do not occur.

A special feature of sprays is their ability to prevent a reaction. If, for example, a patient suffers from a seasonal allergy to the flowering of certain plants (hay fever), then shortly before the start of the period he can start using the spray.

For this allergy, nasal antihistamines will prevent rhinitis from developing.

It is usually recommended to start using a non-hormonal antihistamine nasal spray for seasonal allergies 2-3 weeks before the allergen plants begin to bloom. During this time, under the influence of the drug, the barrier layer in the nasopharynx will increase and its susceptibility to irritating substances will decrease. The sufferer will not feel any symptoms or they will be mild. For the best effect, the doctor may prescribe the patient to take tablets of a similar effect.

Antihistamine antiallergic sprays guarantee relief of symptoms, but they cannot quickly overcome the signs of an allergy that has already begun. Their effect develops gradually. If, when the mucous membrane of the nasal passages is swollen, a vasoconstrictor drug acts within 2-5 minutes, then using an antihistamine in a spray, you will have to wait 20-30 minutes to relieve swelling in the nasopharynx.

For severe nasal congestion, it is recommended to use both types of medication. The vasoconstrictor spray is discontinued after 2-3 days, when the cumulative effect of the antihistamine is achieved.

List of the best antihistamine sprays against nasal allergies:

  • Levocabastine;
  • Aldecin;
  • Cromohexal;
  • Allergodil;
  • Tizin Alerji;
  • Histimet.

Many drugs in this group have contraindications - age under 6 years, pregnancy. Even if the medicine was prescribed by a doctor, you should read the instructions before use. You can instill non-hormonal antihistamine drops for a long time until your health condition stabilizes.

Allergy sprays with cromoglycate (cromoglicic acid)

Cromoglycates stabilize mast cell membranes. It would take quite a long time to explain this phrase, so let’s focus on the properties of these sprays. These drugs appeared a long time ago and are gradually losing their position. However, they have an excellent safety profile. The action is quite weak, the effect does not occur immediately, but they have a cumulative effect. I require multiple doses - 3-4 doses per day. In general, good for mild forms. Relatively inexpensive, even taking into account the large number of “zilch” per day.

  • Cromohexal. For adults and children over 5 years of age, it is recommended to administer 1 dose (one pump) (2.8 mg of cromoglycic acid) into each nasal passage 4 times a day (total - 11.2 mg of cromoglycic acid). If necessary, you can use 1 dose up to a maximum of 6 times (maximum 16.8 mg) per day in each nasal passage.

After achieving a therapeutic effect, the frequency of use of the drug can be reduced and Cromohexal can be used only when in contact with allergens (house dust, fungal spores, pollen). The course of treatment is 4 weeks. Cancellation should be made gradually over 1 week.

Before using the drug for the first time, remove the protective cap and press the sprayer 3-4 times to achieve uniform dosing of the drug. Before using the drug, you should clean the nasal passages, carefully insert the spray nozzle into the nasal passage and, while inhaling through the nose, press the spray nozzle all the way. After using the drug, the spray nozzle should be wiped with a dry, clean cloth and closed with a protective cap.

  • Cromoghlin. Spray - adults and children over 6 years old - 1 dose (2.8 mg) in each nasal passage 4-6 times a day.

Treatment of allergic rhinitis

The main task in this disease is the elimination of allergens. Without this condition, treatment will be ineffective and will only bring temporary relief.

The most effective are hormonal nasal sprays. They necessarily contain a glucocorticosteroid. The most popular according to the rating are:

  1. Nasobek. This is a drug with the active ingredient beclamethasone. Due to the rapid absorption of the drug by the nasal mucosa, swelling is reduced and inflammation is relieved. With a correctly selected treatment regimen, the condition improves significantly within 2-4 days. Used for allergic, chronic rhinitis and as an aid for sinusitis. The cost of this spray is from 180 to 250 rubles.
  2. Nasonex. The active substance is momentozone. An excellent remedy for allergic sinusitis. Used only once a day. This dose is enough to get rid of allergic rhinitis for 24 hours. Can be recommended for children over two years old.
  3. Flixonase. It contains fluticasone. Relieves inflammation of the nasal mucosa, but it can only be used from the age of 12. Contraindicated for people who have recently suffered a nasal injury.
  4. Tafen Nazal. A hormonal drug for intranasal use. Normalizes respiratory function.

Allergy barrier sprays

They are not included in any official recommendations, being, in fact, auxiliary means of limited use. They are safe due to the fact that they contain practically no active medicinal components. Usually contain vegetable oils as additional components.

Local nasal sprays, the contents of which settle on the walls of the nasal cavity in gel form and thereby protect it from the action of various types of allergens.

The cost is comparable to medications, but they are used only as a supplement to them.

  • Nazaval. The active component of the drug is micronized cellulose. Also contains natural peppermint extract.

In adults and children (under adult supervision): 1 spray into each nostril every 6–8 hours. As a rule, the recommended dosage is sufficient to prevent symptoms of rhinitis. It is recommended to repeat injections of the preparations after each blowing of the nose to resume the formation of a protective film.

  • Nazaval plus. Same as Nazaval, only with wild garlic extract.

It is recommended to apply the spray 10-15 minutes before possible contact with the allergen. Nazaval can be used an unlimited number of times.

  • Prevalin. A mixture of vegetable oils and glycerin. Softens the mucous membrane and creates a barrier to contact with the allergen. It can also be used without special restrictions.

1-2 injections into each nasal passage, 2-3 times a day (one dose provides protection for 4-6 hours). Children 6-12 years old should use Prevaln Kids only under adult supervision. Before each use, you must clean your nose.

Features of using sprays

The spray against allergic rhinitis is undoubtedly good for its ease of use. If this is not the first time an adult has encountered an allergy, then he can decide to use one or another spray on his own, based on the experience of past years and his previous consultations with a doctor.

The spray form of the medicine will not allow an overdose, since one injection requires a strictly defined amount of the active substance.

Unlike tablets, medicine in the form of a spray penetrates into the blood minimally, acting only at the site of its use, that is, on the nasal mucosa.

Local application allows the use of some drugs during pregnancy and lactation. The doctor decides which one to choose in your case.

The spray can is convenient to carry with you and use anywhere - the procedure takes just a few seconds.

Combination allergy sprays

The main use is the initial stage of treatment, when symptoms are most severe, in order to subsequently switch to a spray with one component. They usually contain two components: a hormone and an antihistamine.

  • Momat rhino advance. Not to be confused with Momat Rino. Contains hormone and antihistamine. As the manufacturer suggests, it is used at the initial stage of treatment, and then, after two weeks, switch to Momat Rino, i.e. for a purely hormonal nasal spray for allergies. The scheme itself is good, but it hits the pocket pretty hard. Inhalation of the suspension contained in the bottle is carried out using a special dispensing nozzle on the bottle.

1 dose of spray (azelastine hydrochloride - 140 mcg / mometasone furoate - 50 mcg) in each nostril 2 times a day in the morning and evening. The duration of treatment is 2 weeks.

  • Vibrocil. It contains antihistamine and vasoconstrictor components, which greatly limits its use, since, as we wrote above, vasoconstrictor drugs cannot be used for a long time. It can be recommended as an “ambulance”, for example, if you come to visit and there is a cat... Or in the initial period of flowering, when the symptoms are most pronounced.

Adults and children over 6 years of age are prescribed 1-2 sprays into each nasal passage 3-4 times a day. When using the spray, the sprayer should be held vertically, with the tip facing up. Keeping your head straight, insert the tip into the nasal passage, squeeze the sprayer once with a short sharp movement, and remove the tip from the nose and unclench it. During spraying, it is recommended to inhale slightly through the nose.

Topical steroids

The human body has special glands - the adrenal glands - that produce hormones, including glucocorticosteroids. They affect the functioning of the immune system, so their synthetic derivatives are actively used to treat allergic diseases.

Hormones have important properties:

  • relieve inflammation;
  • reduce allergy symptoms;
  • with local treatment they are not absorbed into the blood and do not have a systemic effect.

The products are mainly produced in the form of sprays. The pharmacological effect does not develop immediately, but gradually increases over 1 or more weeks. Therefore, in case of severe swelling of the mucous membrane, it is recommended to combine treatment with vasoconstrictor sprays.

Advice: if you drip any oil drops into your nose a few minutes after administering glucocorticosteroids, you can prevent dry nose, burning and itching. The duration of the course, dosage and choice of dosage form of the drug should be determined by the doctor.

Momat Rino Advance

Elfasept® SEA spray cleanses the nasal mucosa and provides hydration for irritation and dryness associated with a runny nose and allergy symptoms.

Vasoconstrictor drugs

Vasoconstrictors reduce blood flow to mucosal epithelial cells.

This leads to the following effects:

  • swelling disappears;
  • the production of nasal mucus decreases;
  • Breathing becomes easier quickly and permanently.

However, there are also negative sides:

  1. the drugs are absorbed into the systemic circulation, which, with long-term treatment, negatively affects the heart rhythm. This happens especially often in children, since their mucous membranes have a high degree of absorption. The rate of absorption is also affected by the presence of damage to the internal epithelium;
  2. the blood supply to the nasal membranes deteriorates, they become more sensitive to the slightest injury. The work of the ciliated epithelium and the natural movement of muconasal secretions (snot) are also disrupted, which slows down the rate of its removal from the nasal passages;
  3. local immunity decreases as the internal epithelium dries out;
  4. addiction develops (medicinal rhinitis). After the drug stops working, the vessels dilate sharply, and swelling of the mucous membrane also returns, but more pronounced. Each time the period of relief becomes shorter;
  5. In children, drops in any case penetrate the nasopharynx and are swallowed, and once in the stomach, they are absorbed into the blood. Therefore, even a slight excess of the dosage can lead to arrhythmia.

It should be remembered that vasoconstrictors should not be used to treat the disease, but only as symptomatic therapy. You can bury your nose for no more than 7 days in a row. To ensure complete absorption of the drug, it is recommended to first clean the nasal passages with a saline solution.

Xylometazoline

Active ingredient: Xylometazoline.
A colorless, transparent liquid has an average duration of action (up to 8–10 hours). Children under 6 years of age are prescribed a 0.05% solution, 1 injection or 1-2 drops in each nostril 1-2 times a day. Adults – 0.1%, 1 injection or 2-3 drops no more than 3 times a day.

The drug quickly constricts peripheral blood vessels, relieves swelling of the nasal mucosa and restores breathing. The composition includes eucalyptus oil, which prevents overdrying. The medicine acts locally, being slightly absorbed into the systemic circulation.

  • ARVI;
  • allergic rhinitis with nasal congestion;
  • hay fever;
  • Otitis media

Xylometazoline is contraindicated in cases of hypersensitivity, hypertension and arterial hypertension, atherosclerosis, glaucoma, as well as in children under 2 years of age (a 0.05% solution is used with caution under the supervision of a pediatrician). The drug is rarely prescribed to pregnant and lactating women.

  • dryness of the nasopharynx;
  • swelling, burning, sneezing;
  • tachycardia;
  • increased blood pressure;
  • vomit;

Allergy nasal sprays for children

Of the hormonal allergy sprays, the safest are considered to be:

Nasonex (allowed from 2 years);

Momat Rino, Desrinit - sprays containing the same active ingredient are also allowed from two years of age;

Tafen Nazal (allowed from 6 years of age) - also considered quite safe;

Tizin Allergy is allowed from 6 years of age, from antihistamines;

Cromohexal spray is approved from 5 years of age, although it is the safest spray for allergies. Of course, to choose the right drug, it is better to consult a doctor.

The children's dose is usually half the adult dose; children over 12 years of age are usually recommended an adult dose. Use must be under adult supervision.

Reviews:

Nadezhda: After a trip to the allergist, it turned out that I am allergic to dust, I don’t know what to do about it. They prescribed Nasonex - now I take it, it seems to be better, but my nose is running all the time.

Snezhana: I bought Kromohexal, otherwise I don’t have the strength, I didn’t go to the doctor, so we’ll see what happens, but for now I’m using it, it’s a decent remedy.

Anton Leonidovich: Flixonase is hormonal, like all sprays, so you should use it more carefully, allergy sufferers know.

Nasal spray for allergies during pregnancy

One way or another, all allergy medications during pregnancy have their limitations. Therefore, if an allergy appears during pregnancy, it is better to consult a specialist so that he can prescribe medications taking into account the risks for the pregnant woman and the fetus. If you have hay fever (an allergy to pollen) and you are planning a pregnancy, it is better to see your doctor in advance so that he can give you recommendations for the flowering period.

Saline sprays for allergies in pregnant women are used without restrictions.

List of sprays safe during pregnancy

It is necessary to understand that pregnancy is a period in a woman’s life when taking any medications without good reason should be minimized. Doctors advise using prophylactic medications before pregnancy to avoid complications during pregnancy. It is also categorically undesirable to use anti-allergy sprays in the first trimester of pregnancy, as this can negatively affect the fetus. It is undesirable to use this group of drugs while breastfeeding.

In such a situation, it is necessary to constantly undergo examination and consult with an allergist, especially for those mothers who suffer from this disease in a chronic form. In any case, there is no guarantee that the drug will not have any negative effects on the fetus, and tests on pregnant rats indicate that anti-allergy drugs lead to heart problems and underdeveloped limbs in rat pups.

However, drugs are described that, to one degree or another, can be used in such a situation and even during lactation, but their use should be under the full supervision of a physician:

  • Suprastin. A specific drug is used only if the mother's allergies threaten the fetus to a greater extent than the drug described. The use of Suprastin is possible for acute allergic reactions and their treatment in the second and third trimesters of pregnancy;
  • Allertek. It is possible to use the product during pregnancy, but the risk still remains and it is very high;
  • Tavegil. It may be a potentially dangerous drug for the fetus, but in case of urgent need it can be used with the permission of the doctor.

Application of sprays

A new generation spray for allergic rhinitis of any type has a small number of contraindications, as a rule, these are kidney, liver, lung diseases, tumors, viral and fungal infections.

You should also be careful when using a spray for allergic rhinitis for children and pregnant women.

During pregnancy

Antihistamines/non-hormonal drugs are not prescribed to pregnant women.

Hormonal sprays cannot be used in the first trimester, and some drugs (Nazarel, Flixonase) are contraindicated throughout pregnancy. In any case, before prescribing and directly using the medicine, you must read the instructions (it will definitely contain a clause about the possibility of using the medicine during pregnancy and lactation).
